- The pharmacist will review how you are taking your medicines and how you are getting on with any equipment or inhalers if relevant. Appropriate support, advice and information will be given
- Medicines optimisation will be discussed for falls and pain management, as appropriate and recommendations will be made for changes in medication if suitable.
- You will receive ongoing support from the pharmacy team whenever you return to collect your medicine. Your progress towards your health goals will be evaluated and new goals agreed where appropriate
